How Cosopt works in the eye?

Timolol is a beta-blocker and blocks the beta receptors present in blood vessels in the eye. It minimizes the amount of aqueous humor production in the eye ball. Dorzolamide is a carbonic anhydrase inhibitor. Carbonic anhydrase is an enzyme usually liable for the generation of bicarbonate salt in the body. Bicarbonate is required in the production of aqueous humor. Dorzolamide produces an inhibitory effect on carbonic anhydrase and decreases the synthesis of bicarbonate. Thus bicarbonates are not available for generation of aqueous humor and finally the reduction in aqueous humor inside the eye ball causes decrease in intraocular pressure.
